The patient may be eligible for Copay assistance if the patient is starting or receiving Lonsurf and the patient has commercial insurance that covers medication costs for Lonsurf and allows for Copay or coinsurance assistance. The patient must have a current prescription for Lonsurf and meet all other eligibility requirements.

This manufacturer program is not valid for prescriptions reimbursed, in whole or in part, by Medicaid, Medicare, Medigap, VA, DoD, TRICARE or any other federal or state healthcare programs, including state pharmaceutical assistance programs, and where prohibited by the health insurance provider or by law. Specialty Pharmacy Providers and Healthcare Professionals must have patient consent to release patient information to Taiho Oncology and its employees and agents to assist in obtaining Copay assistance.
